Supv Patient Care Coordinator

Overview: Coordinates and monitors all Care Management team activities, provides leadership, coaching, and mentoring to Care Management and Social Works staff members. Responsible for providing leadership and direction for Discharge Planning and Transitions of Care, within the acute hospital. Monitors for quality indicators to assure appropriate social and transitional services are provided to patients and families. Develops and maintains relationships with physicians, nursing supervisors, payers, community resources/ agencies to provide the needs services for indigent, uninsured, and underinsured populations. Qualifications: Education

• Associate's Degree from an accredited school of Nursing Required or
• Master's degree in Social Work and current Social Work licensure in the State of Georgia Required
• Bachelor's Degree from accredited school of Nursing Preferred

• 3 years of experience in care management, medical social work or transitional care management Required

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Current license in the state of Georgia as a Registered Nurse or NLC/eNLC Multistate License Upon Hire Required or
• LMSW - Licensed Medical Social Worker - State Licensure Licensed Master Social Worker (LMSW) in the state of Georgia Upon Hire Required
