PRN Transplant Social Work Job Description:

This person will work 5 days per week supporting both the kidney and liver team with one work from day if needed

Responsibilities:

This person will work 5 days per week supporting both the kidney and liver team with one work from day if needed. Duties will include the following:

1. Complete psychosocial evaluations for transplant candidates and living donors

2. Communicate with patients via phone to assist with prescreens, transplant listing and post-transplant needs

3. Provide coverage for absences

4. Assist both kidney and liver teams with managing caseloads for pre, waitlist, living donor and post-transplant.

Qualifications:

Education

• Graduate of an accredited school

Work Experience

• 2 years of care experience Preferred

Licenses and Certifications

• Graduate of an accredited school
