Job Description

To enhance patient and family experience, proactively guides Heart Center patients and families through inpatient, outpatient, and procedural modalities, coordinating communication between Children’s, the referral institution, and patient family.

Experience

  • 5 years of experience in healthcare setting
  • Demonstrated experience working with patient families
  • Demonstrated experience in care coordination and/or case management

Preferred Qualifications

  • Experience working with critical care patients and families
  • Experience working with a wide range of community resources, referral sources and other professional organizations

Education

  • Bachelor’s Degree in Business or Health Related Field

  • Ability to manage multiple and continually changing priorities and make rapid decisions
  • Ability to display a high degree of professionalism and build effective relationships with patient families, physicians, and other health care providers
  • Team player with strong interpersonal skills, effective verbal and written communication skills, ability to handle conflict, and reach consensus through collaboration.
  • Proven effectiveness and ease in working with diverse groups of varying age, socio-economic, cultural, religious, and sexual orientation.
  • Must have computer experience and proficiency in a wide variety of software, e.g.., Microsoft Office applications and database management

Job Responsibilities

  • Proactively guides Heart Center patients and families through inpatient, outpatient, and procedural modalities, coordinating communication between Children’s, the referral institution, and patient family.
  • Partners with team members to foster an environment that promotes professional development and problem-solving, maintaining and enhancing knowledge of current issues and integrating ideas into practice.
  • Cultivates a working knowledge of appropriate internal and external referral sources. Develops, supports, and maintains existing relationships with a wide range of community resources, referral sources and other professional organizations.
  • Facilitates the intake process, ensuring that patient demographic, contact and insurance information are accurate. Obtains necessary authorization for release of records and monitor record requests for arrival.
  • Schedules and coordinates complex multidisciplinary cardiac consultations based on disease-specific or transplant scheduling algorithms.
  • Maintains assigned databases.
  • Collaborates in collection of data for identification and resolution of flow issues.
  • Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes within Heart Center.
  • Participates actively and contributes to organizational initiatives that impact patient flow and experience, supporting continuous performance and quality improvement.
  • Communicates openly with appropriate leadership team to ensure knowledge of key decision factors.
  • Serves as expert resource for patient placement by ensuring smooth functioning of Heart Center process to reduce wait times and avoid unnecessary patient movements.
  • Maintains positive working relationship and demonstrates excellent customer service skills in all interactions between Heart Center, referring institution, physicians, and other stakeholders.
